Making the Best Use of Your Time
One of
the most frustrating aspects of a dental visit is
the paperwork
that needs to be completed before the you even see a doctor. To better respect the use of your time, we have
provided the
necessary forms for you to print off at home, fill out and bring with
you to your first appointment.
You will need to have JavaScript enabled on your browser, and Adobe Reader to print them. If you have both, simply click on either the "New Patient History" or "Insurance Information" forms in the Menu on the right.
